As we age, our skin slows in its ability to produce collagen. This results in the formation of lines, wrinkles and folds as well as loss of lip volume and definition. Additionally, we experience the atrophy or loss of subcutaneous tissue (fat). The presence of fat under our skin confers the rounded appearance of youth; its absence manifests as the gaunt look of the aged.
Dermal fillers are indicated for correction of wrinkles, fine lines, depressed scars, nasolabial folds (around the nose), marionette lines (around the mouth), tear trough deformity (the under-eye groove) and lip augmentation. They are increasingly being used for cheek, chin and hand augmentation. Collagen, one of the first fillers, has been surpassed with other technologically superior longer lasting ones. Treatment consists of numbing the skin at the treatment site then injecting a small amount of filler. The possible side effects of bruising, swelling and mild redness are very transient and easily managed. Results are immediately appreciated.

Hyaluronic acid is a natural component of skin. It is closely associated with collagen and elastin fibers, all of which give our skin its shape. These injectable gels are synthetically derived from bacteria and are non-allergenic (which eliminates the need for skin testing). Years of use worldwide have shown all to be safe and effective. These fillers are easily injected and yield excellent, natural-looking results. Lip enhancement, treatment of under-eye hollows and erasure of fine lines are the most common indications and results are typically maintained for about a year. However, in the tear trough, two years of correction is usually the norm.

Restylane Silk, Refyne, & RHA-2
The lip and surrounding area is the second most common treatment site for fillers on the face. “Restylane Silk. Refyne and RHA-2 are smooth gels specifically designed to improve natural, youthful definition and symmetry to the lip and mouth area”. Restylane Refyne, Restylane Defyne and RHA’s
These fillers are known for their ‘elasticity’ and ability to move with facial movement. This property has made this an excellent filler-of-choice for use in very mobile facial areas (around the mouth and eyes).
Juvederm Voluma, Restylane LYFT and RHA-4
These are Hyaluronic Acid fillers, but unlike the other HA’s, these HA fillers are ‘heavy-duty’ and are able to be placed deep into the tissues, an area where the other HA’s would fail. The indication for these is for mid-face volumization (cheek augmentation). The results are subtle but powerful in restoring the smooth contours of a youthful face and that oh-so-feminine Ogee Curve of the cheek.

RADIESSE
Radiesse (calcium hydroxylapatite) is a synthetic, bone-like material. It has been used as a filler for years worldwide. More robust than hyaluronic acid, it is an excellent filler for deep folds, scars, and wrinkles. One of the most exciting uses for this great filler is in augmentation of the cheek, chin and hands. Due to its heavy nature, “a little goes a long way,” and correction can be achieved with smaller amounts of this material. Results are typically maintained for 12–18 months.

A Non Surgical Facelift with Stimulatory Fillers

As the field of aesthetic medicine has evolved, so too has our understanding of the aging face. The face-lift has long been revered as the only treatment option for facial aging however, we now recognize that “tight is not always right”, and that loss of volume and deflation in the face plays a critical role in the aged look. In fact, facial volume loss actually precedes skin sagging! Loss of fatty tissues begins in our late 30’s (with the effects of gravity presenting years later). This volume loss cannot be corrected with surgery. A youthful face is devoid of lines and wrinkles but it is also full. With advancing years, the convexities (roundedness) of the youthful face transform into concavities (hollows) of the aged. These concavities cause shadows to form thus producing a gaunt look which may be more aging than wrinkles!
Fortunately, this volume loss can now be corrected with Sculptra. Sculptra (poly-L-lactic acid) is technically a filler substance, but it differsgreatlyfrom hyaluronic acids and Radiesse. When injected, it induces dermal thickening by increasing the amount of collagen in the skin. Due to this unique nature, it is known as a stimulatory filler. Normally several treatments are required and the correction is gradual however the final results are natural looking. This restoration of fullness can turn back the hands of time by 15 to 20 years! Results are very impressive and are sustained for at least twenty-five months and in my experience, even longer.

PDO THREADS
The newest technology to help us restore our youthful appearance is found in a suture material that has been in use for over 30 years. I am referring to PDO (polydioxanone) threads. This absorbable suture exerts its effect in two ways: It is a powerful collagen stimulant and the smooth threads thicken and firm the skin thereby correcting facial wrinkles (such as those seen in the cheeks and upper lip – aka smokers lines). The modified barbed threads can also lift sagging (ptotic) skin. This is truly a marvel since the only option in the past was an invasive surgery with its risks, discomfort, prolonged recovery and high price. By pairing lifting PDO threads with a filler that corrects volume loss, we truly now have a non-surgical face lift!
